I assume you're using an XSL-FO based PDF. Get the file Topic.for from the temp directory. This is a concatenated, FO version of your publication. Open it and have a look at one of the problem images. You should see the spacing as attributes of an fo:block element. You can decide what to do from there.

Instead of recreating the text inside your graphic, it is much easier and cleaner (and easier to translate) if you use callout numbers.
